Vaccination evidence can either be found on the Australian Immunisation Register by logging in to your My Health Record portal.
You will be unable to apply for a shift if you do not meet the minimum vaccination requirements.
If you are unable to be fully vaccinated due to a medical contraindication, upload your evidence and it will be reviewed by our onboarding team.
An Immunisation Medical Exemption or a medical contraindication to vaccination is a medical condition or risk factor in a worker that makes receiving a specific vaccine potentially harmful, as assessed by a suitably qualified medical practitioner.
Discuss with your doctor who will provide advice and assess if you have a medical contraindication.
Have your doctor complete and submit an Immunisation medical exemption form (IM011) to the assessor in your local health district to support your application.
The only acceptable evidence of a medical contraindication is an Australian Immunisation Register (AIR) Immunisation medical exemption(IM011)
